Assistance with Daily Life (Includes Supported Independent Living)

A​ssistance with and/or supervising of personal tasks of daily life to enable the participant to live as autonomously as possible. (ie household decision making, personal care, domestic tasks ) Supports are provided individually to participants and can be provided in a range of environments, including but not limited to the participants own home and can include short term accommodation, ie respite care.

Transport

T​ransport enables participants to access the community for educational, recreational and vocational purposes. Transport funding is limited to those who cannot use public transport due to their disability and who use specialised transport to access supports that help them achieve their goals.

Consumables

Consumables are a support category available to assist participants with purchasing everyday use items. Coordination of Supports:​ Assists with strengthening a participants ability to design and build their supports to link a broader system of supports across a complex service delivery environment.

Assistance with Social and Community Participation

A​ support that enables participants to engage in community, social or recreational activities. They can be provided in a centre or community setting at a standard or higher intensity rate.

Assistive Technology

Assistive equipment for recreation, assistive products for household tasks, personal care and safety. Low cost/low risk products, aids or equipment supports that assist participants to live independently or assist a carer to support the participant.

Home Modifications and Specialist Disability Accommodation (SDA)

H​ome modifications to enable participants to live as independently as possible or to live at home safely. Includes the design, construction, installation and/or changes to non-structural components on the building and installation of fixtures and fittings needed to achieve this.

Coordination of Supports

Improved living arrangements

Assistance and to support a participant in obtaining and/or retaining appropriate accommodation assistance with applying for a rental tenancy.

Increased Social and Community Participation

Activities that assist in building skills and independence in accessing the community and include capacity building, mentoring or peer support and individual skill development.

Finding and Keeping a Job

W​orkplace assistance enabling a participant to successfully obtain/retain employment in the open or supported labour market. Includes transition to employment, work, work skills, employment preparation and support.

Improved Relationships

I​mproved relationships supports includes specifically designed positive behavioural support strategies for a participant, their family and support people to achieve elimination or reduction in behaviours of concern.

Improved Health and Wellbeing

Activities to increase and improve physical capacity and health. These activities support, maintain or increase well-being and/or physical mobility. ie exercise physiology or personal training.

Improved Learning

The provision of skills training, advice and assistance with arrangements and orientation when a participant is moving from school to further education.

Improved Life Choices

Engaging registered plan managers who undertake financial assistance for the payment of providers on behalf of a participant and to assist them to develop skills and the ability to manage the tasks associated with their plan.

Improved Daily Living Skills

A​ssessment, training, development and/or therapy to assist in the development of or increase in skills and improved capacity for independence and community participation. Service can be delivered in groups or individually and includes training for carers in matters relating to caring for a person with disability.
